How do I schedule an appointment?

You can request an appointment online using our appointment form, or call us directly at (941) 867-9362 during office hours. We offer same-day sick visits, call early to reserve your spot.

What should I bring to my first visit?

Please bring a valid photo ID, your insurance card, a list of current medications and allergies. All patient forms are completed online through our secure patient portal, our staff will guide you through the process.

Do you offer telehealth appointments?

Yes! We offer secure video visits for follow-ups, medication refills, chronic care check-ins, and minor illness consultations. Call or request online to schedule a telehealth visit.

How do I request a prescription refill?

For routine refills, you can submit a request through our patient portal, call our office, or ask your pharmacy to send a refill request directly to us. Please allow 2-3 business days.

Do you offer care in Spanish?

Yes, we proudly offer bilingual care in English and Spanish. Please let us know your language preference when scheduling.

What if I do not have insurance?

We welcome self-pay patients and offer transparent, affordable pricing. Initial visits are $85 and follow-up visits are $60. Payment is due at the time of service. Please call our office if you have any questions.
